Informed Consent: I, (name listed above), declare that I intend to use the Community Weight Room operated by the Oliver Parks & Recreation Society and I understand that each person, (myself included), have different capacities for participating in such activities, facilities, programs, and services. I am aware that all activities, services offered at the Community Weight Room are self-directed in nature. I assume full responsibility before, during and after participation, for my choices to use or apply, at my own risk. I understand that part of the risk involved in undertaking any physical activity is relative to my own state of fitness or health (physical, mental, or emotional) and the awareness, care and skill with which I conduct myself in that activity. I acknowledge that my choice to participate in any activity or service brings with it the assumption by me of those risks or results stemming from this/these choice(s) and the fitness, health, awareness, care and skill that I possess and use. In addition, I understand that I am free to withdraw from, reduce or modify my involvement in this activity and I realize that I should do so upon recognition of any signs of transient lightheadedness, fainting, chest discomfort, leg cramps, nausea, etc. I further understand that the activities, programs and services offered by the Oliver Parks & Recreation Society are sometimes conducted by personnel who may not be licensed, certified, or registered instructors or professionals. I accept the fact that the skills and competencies of some employees and/or volunteers will vary according to their training and experience and that no claim is made to offer assessment or treatment of any medical or physical disease or condition by those who are not duly licensed, certified or registered and herein employed to provide such professional services. In addition, I acknowledge that I have inquired about the nature of any activity, program or services that I am not completely familiar with and I have been informed or any inherent risks. I declare that I have read, understood and agree to the contents of this INFORMED CONSENT AGREEMENT in its entirety. Weight Room Code of Conduct: I understand and agree that each time I scan my key fob and enter the facility I am declaring that I will follow the code of conduct for the Oliver Community Weight Room: • I will ensure that my membership is always in good standing. • I will not use my key to allow entry to non-members or open the door to allow entry for non-members. • I will respect the equipment in the weight room by using equipment properly, putting things back in their place, wiping down equipment after use and informing staff or volunteers when something is not working properly. • I will remove outside footwear and put on a pair of clean shoes. • I will dress in attire appropriate for working out in a public facility. • I will be respectful of staff, volunteers and other weight room users. • I will respect the 20-minute time limit on cardio machines during peak/busy times. • I will strive to always use the weight room space and equipment in a safe, appropriate way that will further my physical fitness. • I will strive to support the core values of Oliver Parks and Recreation: Safety, Health, Commitment, Accountability, Fun, Respect, Service Excellence COVID-19 Guidelines: I understand and agree that each time I scan my key fob and enter the facility I am declaring that: • I will not enter the weight room if I am sick or required to self-isolate • I will maintain physical distancing (6ft) at all times in the weight room • I will use disinfectant wipes to clean equipment after each use • I will wash my hands well before and after using the weight room • I understand that the Weight Room Attendants have the authority to cancel my visits to the facility if I do not follow these guidelines.

Please allow 2-4 days for processing. You will be contacted about your application by a staff member. Individual Applicants are eligible for up to $150 of funding assistance per calendar year to be used for programs and services through Oliver Parks & Recreation.

2. Do you feel pain in your chest when you do physical activity?

3. In the past month, have you had chest pain when you are not doing physical activity?

4. Do you loose your balance because of dizziness or do you ever loose consciousness?

5. Do you have a bone or joint problem (for example, back, knee or hip) that could be made worse by a change in your physical activity?

6. Is your doctor currently prescribing drugs (for example, water pills) for you blood pressure or heart condition?

7. Do you know of any other reason why you should not do physical activity?

If you answered yes to one or more questions Talk with your doctor by phone or in person BEFORE you start becoming much more physically active. Tell your doctor about any of the questions that you answered YES.

I certify that this document is true and accurate and I agree to advise the Oliver Parks & Recreation staff, in writing, of any change in my medical condition. I understand that unless the Oliver Parks & Recreation Department hears from me otherwise, they will assume that all medical information is unchanged from the date of this agreement.
